Do you have a current Hen House Rewards or Chopper Shopper card? If you do, you can earn money for The Heartland Book Bank every time your card is scanned. The Community Rewards Program is a new program sponsored by the BALLS Food Stores to aid worthwhile community groups in their fundraising activities. Through this program, BALLS Food Stores will donate cash equal to 1% of the net purchases you make at any Hen House Market or BALLS Price Chopper in the metro area, from Sept. 1 through May 31 every year. All you have to do is use your qualified "Hen House Rewards" or "Chopper Shopper" card when you make the purchase. If you are a new Hen House or Price Chopper customer, just fill out the required form (click here to download form) and return it to the Heartland Book Bank to receive your card's). If you are a current Hen House Rewards or Chopper Shopper cardholder, just fill in your name, address and phone number on the form. Then, in the white space at the bottom of the form, list the card numbers of the Hen House Rewards and/or Chopper Shopper cards you wish to be included in the program. These card numbers all start with 40, are either 11 or 12 digits long and are normally located near the bar code on the card or key tag. (Some Chopper Shopper cards may not have the bar code number in readable form. If your Chopper Shopper card is one of these, you can take your card to the Customer Service Office at the nearest Hen House Market or BALLS Price Chopper. They will be happy to read the bar code for you.) After you have completed the form, return it to Heartland Book Bank  at the address below. Then use your newly qualified Hen House Rewards or Chopper Shopper card each time you shop at a Hen House Market or BALLS Price Chopper anywhere in the metro area!   Your card must be presented at the time of the transaction to qualify for the 1% reward on your net purchase. If you have friends or family who wish to participate, they also can fill out an enrollment form and become a qualified cardholder.
